New Patient Forms
Click the link at the BOTTOM of this page to download the Sign Up Form/Case History Form that you will need to bring to your first visit. Go to About Us and click on a doctor name to learn more about each one. To make an appointment, please call 303-449-3103 and speak with one of the Receptionists-Chiropractic Assistants.
INITIAL VISIT:
* Duration is approximately 30 minutes. Come 5 minutes early to your appointment to fill out paperwork if not previously downloaded and completed (PDF link at bottom of page).
* X-Rays and MRIs are not required. If you have them please feel free to bring them to your first appointment. The doctor may or may not refer you out for x-rays/MRI.
* You and the doctor will review your case history, discuss your prior health care and reason for coming to this office, conduct an assessment and clinical exam and an adjustment. You will also learn more about the chiropractic care we offer and get to know the doctor.
* Your care plan and follow up visits will be determined by you and your doctor.
Once you are a patient at Swan Lake it is our hope that you would see and benefit from each doctor in order to create a well-rounded experience for you.
